Tuberculosis (TB) is an infection caused by Mycobacterium tuberculosis. It is a disease that has been known since antiquity but remains prevalent today, despite the availability of effective anti-TB antibiotics. In recent decades, the increasing prevalence of drug-resistant TB has complicated treatment. Untreated TB is deadly and carries a high mortality rate (70% for untreated smear-positive cases). The World Health Organization (WHO) reports that TB remains one of the top ten causes of death worldwide and caused 1.7 million deaths in 2015 alone. Early diagnosis of TB is essential, both for treating the infected patient and for preventing spread of TB to the community.

Carotid body and vagal paragangliomas, although considered indolent tumors, represent a challenge for the treating physician. This is mainly because of their peculiar localization, in close proximity with important anatomical structures. In addition, there is no chemotherapy available for these tumors, the role of radiation therapy is debated, and the only successful therapy is surgery. However, to achieve the best treatment goals, it is fundamental for the professional caregiver to master not only the clinical and surgical procedures but also the genetic backgrounds and the histopathological features of these tumors. We provide in this chapter a comprehensive review of the above mentioned aspects, with the aim to address the complexity of these tumors with a multidisciplinary approach.

In this chapter, we provide a focused review on a highly challenging subset of head and neck paragangliomas, that is, those arising from the skull base (jugulotympanic region). Presently, these tumors can be cured only with surgery, which must be performed in highly specialized skull base surgical centers. We review here the clinical presentation, diagnostic workup, classification, and surgical management of these rare but important tumors, together with currently available evidence concerning genetics, developmental origin, and pathology.

Tumors of smaller sizes have good immediate postoperative FN results and recover well at the end of 1 year while more than 3 cm have poor outcomes and recover poorly at the end of 1 year. When the VSs reaches more than 1 cm, the HB I and II outcomes drop significantly.

Owing to the indolent nature of tympanojugular paragangliomas, they often present late with cranio-temporo-cervical extensions rendering them inoperable in the past. However, significant developments in the last couple of decades have changed this scenario. A thorough anatomical and surgical mapping of the skull base and descriptions of various rational approaches, in addition to technological improvements in neuromonitoring, neuroanesthesia, and neuroradiology, have made surgical removal of skull base tumors technically feasible and safer.

In patients with good facial nerve function (HB grade I-II), a wait-and-scan approach is recommended. In cases where the facial nerve has been interrupted during surgery, the cable nerve interpositioning technique is a convenient and well-accepted procedure for immediate restitution of the nerve.

Surgical technique and results of cable graft interpositioning of the facial nerve in lateral skull base surgeries

Cable graft interpositioning is a convenient and well-accepted procedure for immediate restoration of the FN. The study results, over a large number of patients, showed that the stitch-less fibrin glue-aided coaptation technique yields good results. The best possible postoperative result achieved was an HB Grade III. The chances of a good postoperative result increase when FN function is normal preoperatively. Slow-growing tumors of the cerebellopontine angle had a favorable outcome after grafting.

The petrous apex is a pyramid shaped part of the temporal bone that is surrounded by the brain, complex neurovascular structures, and the organ of hearing. Diseases affecting the PA not only tend to have serious morbidities due to the affliction of important structures in the vicinity, but they are also difficult to approach surgically due to the deep location in the skull base. The presence of the internal carotid artery (ICA) and the facial nerve in the close vicinity adds to the surgical challenge. The treating surgeon must have a thorough understanding of the anatomy, clinical pathology, and skull base approaches to effectively deal with such lesions.

Radical disease clearance must take precedence over hearing and FN preservation in PBCs. Active FN management, including rerouting, end-to-end anastomosis, and cable nerve grafting, routinely come to play in the surgical management of PBCs, and the postoperative FN results after such interventions can be satisfactory in most cases.

Lateral skull base approaches have an advantage over other approaches in the management of benign tumors of the parapharyngeal space due to the fact that they provide excellent exposure with less morbidity. The use of microscope combined with bipolar cautery reduces morbidity. Stenting of internal carotid artery gives a chance for complete tumor removal with arterial preservation.

The rate of residual cholesteatoma tends to decrease as age increases. The type of cholesteatoma, acquired or congenital middle ear, were not statistically related to the incidence of residual cholesteatoma. Hearing analysis showed that hearing recovery was excellent with canal wall up procedures and remained stable over five years.

A new classification of temporal bone osteomyelitis has been proposed. Bacterial cultures must be performed in all patients. Antibiotic therapy is the treatment of choice. Surgical intervention is necessary in the presence of severe pain, complications, refractory cases, or the presence of bony sequestra on radiology.

A literature review of the results of RT did not conclusively prove that it was effective as a primary modality of treatment for temporal bone paragangliomas. In elderly patients with advanced tumors, a wait-and-scan approach is recommended, and RT is initiated only when fast-growing tumors are detected. There is insufficient evidence in literature to suggest that RT could be an effective alternative to surgery in Class C and D tumors.
